DECLARATION

It is illegal to tattoo any person(s) under the age of 18 in the UK. I confirm that I am at least 18 years of age. I acknowledge that photo identification may be required and a photocopy of my ID will be retained by the studio. 

The medical information I have provided is accurate and truthful. I acknowledge that any ill effects I may suffer as a result of NOT declaring accurate medical information is entirely my fault. I will inform the tattoo artist/piercer if I; (or anyone in my household) has been experiencing any Covid-19 symptoms on the day of my appointment. I understand that I will be refused entry into the studio if I; (or anyone in my household) has been diagnosed with Covid-19 in the last 14 days or I; (or anyone in my household) is showing symptoms of Covid-19 on the day of my appointment.

To the best of my knowledge, I am not pregnant or nursing.

I acknowledge that, it is not reasonable/possible for my tattoo artist/piercer or any representatives of the tattoo studio, to determine whether I might have an allergic reaction to the ink or any of the processes involved in my tattoo or piercing. I agree/accept that such a reaction is possible.

I acknowledge that infection is always possible as a result of getting a tattoo/piercing particularly in the event I do not take proper care of my tattoo/piercing. I have received proper instruction on after-care from my tattoo artist/piercer and/or a representative of the tattoo studio.
